This pull request fixes nested handoff history producing an incomplete Programmatic Tool Calling replay. It treats a hosted program, its program-owned child items, and its program output as one indivisible transcript when constructing the handoff summary, while preserving the complete raw sequence in session history.
It also adds an integration regression covering PTC execution, nested handoff model input, final completion, and SQLite session persistence.
