I encountered a similar issue where images/videos failed to generate thumbnails. The solution was to delete records in the oc_previews table that existed but didn't actually have thumbnails, and then re-execute php occ preview:pre-generate -v.

It might also be necessary to clear records in the oc_filecache table that contain the query select * from oc_filecache t where t.path like 'appdata_%_preview%';.
