[Access] Document tool and prompt aliases for MCP server portals

Summary

Expands the existing "Rename tools with aliases" subsection in the MCP server portals docs with dashboard instructions, an API example, reset steps, end-user impact notes, and a caution about upstream renames. Also adds a changelog entry and screenshots for the feature.

Replaces #30787.
