Implements MG-04, a paper-document-style interactive form for the SIS-03 cyber insurance scenario. Players assess four warranty rows (W-03, W-07, W-09, W-12) against forensic evidence, selecting Compliant/Arguable/Breached per row with optional evidence notes. On submit, writes warranty_checklist_complete, ins001_assessed, and ins003_assessed globals and unlocks Eleanor Vance's warranty discussion branch. Includes Eleanor Ink fallback write.
Known deferred item (MM-02): The Evidence Archive currently unlocks after policy + FDP review rather than after checklist completion as planned; the globals write correctly but the archive eventMapping wiring in scenario.json.erb is a future pass.
Untestable without dependencies: The submission gate requires warranty_evidence_reviewed = true, which is set by opening Exhibit C. Full end-to-end flow cannot be exercised until the evidence packets are built.
