NBRS-4983: Update APP5 medication and allergy wording
Updated the wording from "MAY be used" to "IS used" to provide clearer guidance on the resource's intended use and reduce ambiguity.
Replaced “novel medications” with “medications administered or prescribed during the latest assessment and not available on existing records”. Replaced “novel allergies” with “new allergies”. Updated MedicationStatement guidance from “MAY be used” to “IS used” to clarify the intended use of the resource and remove ambiguity.

Updated the formatting of the medication and allergy requirements by bolding must to align with other mandatory requirements in the implementation guide. No changes to the requirement wording or functionality.

Updated the formatting of the medication and allergy requirements in the Scope & Requirements page to emphasise mandatory requirements by bolding must.
Updated the Referral Payload page to emphasise that only new medications and new allergies identified during the sender's encounter should be included in the referral by bolding new.
Updated MedicationStatement guidance from MAY to IS used, clarifying that MedicationStatement is the expected resource for carrying new medications prescribed at the encounter.
Updated AllergyIntolerance guidance from MAY be used to is used, clarifying that the resource is used to carry new allergies confirmed at the sender's encounter.
