What I found
Several itemids in icu/d_items share the same label, linksto, category and param_type, so nothing in the dictionary says how they differ. I found these through a keyword search, so the list may be incomplete. The query below lists them all.

For CAM-ICU, the sets look like successive versions of the charting form:
- 228300 to 228303 appear only in about 2008 to 2013. Their values carry form instructions, such as "No (Stop - Not delirious)" and "Yes (3 or more errors, then Continue)".
- 228335 to 228337 appear mostly in about 2011 to 2016.
- 229324 to 229326 appear from about 2014 on. 229325 also takes the values "Language Barrier" and "Preexisting Advanced Dementia".
- 228334 (Altered LOC) is charted alongside both of the later sets.
The years are approximate, from anchor_year_group.

Could the documentation, or d_items itself, say what distinguishes these itemids, for example the form version or the period each was in use? It would also help to confirm whether they should be combined when analysing the concept across the whole date range. Without that, anyone who picks the itemid by label silently loses years of data.
