Summary

• Refine issue-triage instructions and prompt-review guidance around task applicability, conflicting requirements, and actionable follow-up tests.
• Add screenshot documentation to the README and user guide; relocate documentation images outside the VSIX runtime assets.
• Document screenshot placement and Marketplace image-link behavior.

Validation

• Full local build: 296 passed, 1 failed, 2 skipped. The existing Windows failure remains in test/verification.test.js for separate restricted-container cleanup (error versus expected passed).
• All 55 local documentation links and 5 screenshot references resolve.
• All 12 moved screenshots match their originals byte-for-byte.
• Actual vsce collection confirms screenshots are excluded and README image links are rewritten to GitHub-hosted URLs.
• Saved comparison export remains local and is not included.

This PR does not rewrite published history or release tags.
