[Feature]: Hide disabled providers from model selector and option to reorder providers

Problem or use case

Hide disabled providers from model selector. For example, I disabled Grok here and there is no point in showing them here. It's poor use of space. Also, it would be great if we can get an option to reorder them.

It's poor use of space and they're distracting as they're appearing even though I specifically disabled them in settings.

Disabled provider instances are now omitted from the model picker, and model ordering can be configured in provider settings. The smallest useful scope requested here is implemented.

This was closed as part of a large repo-wide maintenance sweep. If you think it's still relevant, please reopen.

I understand that models can be reordered but I was specifically talking about reordering providers.

For example, ChatGPT & their models are always at the top and OpenCode & their models are always at the bottom based on current order. I was asking for the ability to reorder those.
