Stuck in Plan Mode - cannot switch to Build mode

Description

I am unable to switch from Plan Mode to Build Mode. The mode toggle does not appear in the UI, and typing /build or /mode build in the chat does not switch the mode.

Steps to reproduce

1. Open OpenCode
2. The app starts in Plan Mode (read-only)
3. Try typing /build or /mode build in chat
4. Try restarting OpenCode multiple times
5. Mode remains stuck in Plan Mode
