• school (this should be a dropdown/select that pulls schools from https://github.com/MLH/mlh-policies/blob/main/schools.csv with an option for "other" and them to type)
• major
• expected graduation year
• email
• date of birth
• food allergies (should have some common ones as a multiselect with an option for other / more info)
• t-shirt size (in adult sizes only - needs to be specified)
